Who we are
Stemming from Baba's Magical Christmas, Chaffee Cares was created to address needs in the community beyond the holiday season. We strive to fill in the gaps where other public programs are limited or unable to assist. Chaffee Cares works closely with and accepts referrals and applications from third party professionals that work directly with local children, adults, seniors, and families in Chaffee County. Using a simple application process and the direction of the board, we strive to quickly help those in need and are dedicated to a 24 hour turnaround time in most situations. By relying on the recommendation of third party professionals and delivering payment directly to the end vendor, there is no direct contact with the individual in order to protect their privacy.
All of our expenses are covered by a core group of volunteers so that 100% of all donations received go directly to those in need. We are a 501c3 which allows all donations to be tax deductible.

There is plenty of parking on F Street and we have reserved parking at the back of the building. Soda fountain treats, over the counter drugs, vitamins and supplements, and merchandise are still available with just a phone call to our staff of at 719-530-4790. We have also implemented online ordering for our entire soda fountain menu and build your own sundae kits to enjoy creating your own delicious treat at home with your family. Heart of the Rockies Regional Medical Center is proud to provide personalized medical care close to home. First established in 1885 as the Denver & Rio Grande Railroad Hospital, HRRMC initially catered to railroad employees and their families during the great western expansion. The railroad company sold the hospital in 1962 to a group of local community leaders. For 14 years, the Salida Hospital, as it was then known, was run as a private entity.
In 1976, the local investors were repaid when the Salida Hospital District was formed, making it a publicly owned institution. In 1989, the Salida Hospital was officially renamed Heart of the Rockies Regional Medical Center to better reflect the offered services. Our facility now serves the visitors to and residents of the Salida Hospital District, which includes Chaffee, western Fremont, and northern Saguache counties.
Fitness for Function provides physical therapy and fitness services in Poncha Springs. Fitness for Function is owned and operated by Leslie S Duran, MSPT, GCS. Leslie Duran is a board certified geriatric clinical specialist. She also is a certified personal trainer and group exercise instructor. Leslie also teaches group exercise, including Parkinson’s Wellness Recovery and provides personal training services at Kim’s Gym in Salida. We are accepting new clients and are in-network with Medicare.

Located in Cañon City, the Solvista Health Primary Care Clinic specializes in patient-centered, comprehensive and preventative healthcare for the entire family. The Chaffee County Health and Human Services departments work together to advocate and provide for the health and wellness of our citizens. We provide responsive programs for the entire community, young and old. We believe the people of Chaffee County are the safest, healthiest & most self sufficient in Colorado.
Long Term Care (Nursing Home) This is a program for person needing help to pay for services in a medical facility for more than 30 consecutive days. These persons have to meet a medical assessment.
Long Term Care (Home and Community Based Services) This is a program for person needing help to pay for services in their home for more than 30 consecutive days. These persons have to meet a medical assessment.
Medicare Savings Program (QMB, SLMB, QI-1) These programs may help persons receiving Part A (Prescriptions are not a covered benefit.)
Medicare Part D–Low Income Subsidy (LIS) This program may help person receiving Medicare Part D with their premiums, co-pays and deductibles.
Supplemental Security Income (SSI) Medicaid This program provides medical benefits for anyone receiving SSI. (No application needed for this program)
Old Age Pension-State Medical Program This program may help person receiving Old Age Pension Financial Assistance.
